Mayfield's Initial Care Department is available Monday through Friday, 9:00 a.m. to 5:00 p.m.,
to help patients get the treatment they need quickly and efficiently.

As a new patient, you will be introduced to our services in four easy steps:
Questionnaire. Download the 2-page Chiari Pre-screening Questionnaire. Send the completed questionnaire and copies of any written radiology reports to chiari@MayfieldChiariCenter.com or fax 513-475-8664. Within 3 business days, you will receive a call from either a Nurse or Initial Care Specialist.
Medical History. A medical history interview will be taken over the phone. You will be asked to submit recent X-rays, MRI/CT scans, or other related
test results. You may be asked to deliver the information to one of our offices. We can also assist you in obtaining the necessary test results.
Priority Consult. The neurosurgeon reviews your medical and diagnostic information and provides treatment and testing recommendations. At that time, someone from our Care Coordination Department will contact you to discuss the surgeon’s recommendations.
Appointment. An appointment with a Mayfield neurosurgeon will be scheduled if needed.
If you've had previous Chiari surgery and would like us to review your records, please follow these steps:
Questionnaires. Download and complete both the Chiari Pre-screening Questionnaire and the Chiari Post-surgery Questionnaire.
Medical Records. Please mail the items listed below to the Mayfield Chiari Center.
- Completed Chiari Pre-screening and Post Surgery Questionnaires
- Your pre-op and post-op MRI scans and cine MRIs (preferably on a CD)
- Operative reports (surgeon's dictation of the surgery)
- Copy of your insurance card, front and back
- Note, please make a copy of all materials for yourself
Medical History. Within 1 week of receiving all your medical records, an Initial Care Specialist will call you to complete a medical history interview over the phone.
Chiari Team Review. Your case will be reviewed and discussed by our Chiari team. Doctors from different specialties will review and discuss your diagnostic imaging, operative reports, and medical history. A report will be sent to you and your physician regarding a possible consultation at our Center.
Appointments for MRI & Cine MRI
A referral from your primary care physician, or a specialist, is required for imaging services at the Mayfield Imaging Center. Appointments are available at the following times:
Mon., Wed., Thu., Fri. 7:30 am - 8:00 pm
Tuesday 7:30 am - 6:00 pm
Saturday 7:30 am - 3:00 pm
